Do you think there is any hope for my home or should I tear it down?

My house is built on a slab foundation, there is no crawlspace or basement. I have been noticing that my house is starting to get a noticeable slant to it and the floors are a little uneven on the top floor. Is there anything I can do for the house or should I tear it down and have it rebuilt?
